Job Description:
A company is looking for a Population Health Director to build
and advance its quality of care product and CCM capabilities.
Key Responsibilities
Support senior Clinical leadership in defining and scaling the
Quality-of-Care/CCM product roadmap
Develop and oversee the product's roadmap, ensuring deliverables
meet business needs
Collaborate with cross-functional teams to translate product needs
into actionable requirements and strategies
Required Qualifications
Bachelor's Degree required; advanced degree preferred (Clinician
preferred)
Minimum of 5 years related technical and professional
experience
Proven track record in program planning and launching successful
products
Knowledge of healthcare industry, particularly value-based care
programs and quality reporting codes
Technical savvy and ability to analyze data for strategic
decision-making
